What you'll be doing

• Provide physical therapy services to patients in home health settings
• Conduct assessments and develop individualized treatment plans
• Manage a caseload of 4-5 patients per day
• Collaborate with healthcare professionals to ensure optimal patient care
• Document patient progress and adjust treatment plans as necessary

What we need to see

• Active California Physical Therapist License
• At least 1 year of experience as a Home Health Physical Therapist
• California driver's license and auto insurance
