Registered Nurse-Transplant

Join our team as a Registered Nurse in the Transplant department, where you'll play a crucial role in patient care and coordination.

What you'll be doing

• Coordinate all aspects of patient care, including record review and medication management
• Make referrals for necessary services and consults
• Act as a liaison between clinical staff, physicians, and administrative teams
• Assist with patient discharge planning and education
• Provide support to patients and physicians in both clinic and hospital settings
• Carry out patient care orders as directed by physicians

What we need to see

• Active RN license in California
• Current BLS certification
• At least 1 year of nursing experience
• Experience with Epic Phoenix
• Outpatient experience in Epic
