Registered Nurse-Home Health

Join our team as a Registered Nurse in Home Health, where you'll provide compassionate care to patients in their homes.

What you'll be doing

• Assess, coordinate, and deliver in-home nursing care
• Provide direct patient care and education to patients and families
• Develop and manage interdisciplinary plans of care
• Foster collaborative relationships with patients, families, and healthcare providers
• Adhere to policies and regulations to ensure quality patient experiences
• Engage in continuous professional growth and development

What we need to see

• Active California Registered Nurse license
• Current BLS certification
• Experience using Epic software
• At least 1 year of nursing experience
• Ability to work every other weekend
• On-call availability for 1 day per month
