What you'll be doing

• Conduct home visits for physical therapy services
• Manage a caseload of 5-10 patients per day
• Document patient progress and treatment plans
• Participate in periodic office meetings
• Travel up to 75 miles from the home clinic

What we need to see

• Licensed Physical Therapist
• At least 1 year of adult home health experience
• CPR certification
• BLS certification
• Reliable transportation with a current driver's license and agency-required liability insurance
• Willingness to travel up to 75 miles from home clinic
