Therapy/Rehabilitation-PT Home Health

Join our team as a Physical Therapist in Home Health, where you'll provide essential care to patients in their homes.

What you'll be doing

• Conduct home visits for patient assessments and therapy
• Participate in periodic office meetings
• Manage a caseload of 5-10 patients per day
• Document patient progress and activities
• Travel between patient homes, with mileage reimbursed

What we need to see

• Current CPR and BLS certifications
• Minimum of 1 year of adult home health experience
• Reliable transportation with a current driver's license
• Agency-required liability insurance

Ways to stand out from the crowd

• Experience with HCHB EMR systems
• Willingness to travel up to 75 miles from home clinic
