Join our team as a Physical Therapist in Home Health, where you'll make a meaningful impact on patients' lives in their own homes.

What you'll be doing

• Conduct patient assessments and develop individualized care plans
• Utilize various equipment and devices for patient rehabilitation
• Provide education to patients and families on home exercise programs
• Document patient progress and maintain accurate records
• Perform skilled home visits, averaging 6-8 per day
• Ensure safety and injury prevention during therapy sessions

What we need to see

• Active state license as a Physical Therapist
• Current BLS certification
• Minimum of 1 year of home health experience
• Experience with assessment in home health
• Valid driver's license and car for home visits
