Join our team as a Physical Therapist in Home Health, where you'll provide essential care to patients in their homes.

What you'll be doing

• Conduct home visits for patient assessments and treatments
• Document patient progress and maintain accurate records
• Collaborate with healthcare teams during periodic office meetings
• Manage a caseload of 5-10 patients per day
• Travel up to 75 miles from the home clinic for patient visits

What we need to see

• Active Physical Therapist license
• Current CPR and BLS certification
• Minimum 1 year of adult home health experience
• Reliable transportation for home visits
