Therapy/Rehabilitation-PT Home Health

Join our team as a Physical Therapist in Home Health, where you'll provide essential care to patients in their homes.

What you'll be doing

• Conduct home visits for patient assessments and therapy
• Document patient progress and treatment plans
• Travel to multiple patient locations within a 75-mile radius
• Participate in periodic office meetings
• Manage a caseload of 5-10 patients per day

What we need to see

• Active Physical Therapist license for Home Health
• Current CPR/BLS certification
• Minimum 1 year of recent adult home health experience
• Experience with PointCare/HCHB EMR system
• Willingness to travel up to 75 miles for patient care
