What you'll be doing

• Conduct home visits for therapy and rehabilitation
• Manage a caseload of 5-10 patients per day
• Document patient progress and activities
• Participate in periodic office meetings
• Travel up to 75 miles from home clinic for patient visits

What we need to see

• Active Physical Therapist Assistant license
• CPR and BLS certification
• Minimum 1 year of adult Home Health experience

Ways to stand out from the crowd

• Reliable transportation with current driver's license and agency-required liability insurance
• Experience with HCHB EMR system
• Willingness to travel up to 75 miles from home clinic
