Social Services-LMSW (Licensed Masters of Social Worker)

Join our team as a Licensed Masters of Social Worker (LMSW) and make a difference in home health care in the Portland area.

What you'll be doing

• Provide skilled home health services to patients in East Portland and surrounding areas
• Manage a caseload of up to 6 patients per day with a 1:1 ratio
• Utilize Oasis for patient documentation and charting
• Ensure compliance with facility policies and procedures
• Travel to patient locations with minimal drive time

What we need to see

• Active LMSW licensure in Oregon and Washington
• Current BCLS certification
• At least 1 year of skilled home health experience
• Experience with Oasis documentation
