Registered Nurse-Case Manager

Join our team as a Registered Nurse - Case Manager, where you'll play a vital role in patient care and coordination.

What you'll be doing

• Perform care coordination and discharge planning for patients
• Manage a caseload of patients with a typical ratio of 1:25
• Collaborate with healthcare teams to ensure optimal patient outcomes
• Provide support and guidance to patients and their families
• Participate in weekend shifts as needed, typically one full weekend every four weeks

What we need to see

• Active California state nursing license
• Minimum of 2 years of acute medical Care Manager experience
• Experience in care coordination and discharge planning
