Registered Nurse-Hospice

Join our dedicated team as a Registered Nurse in Hospice, where you will provide compassionate care to patients in their homes.

What you'll be doing

• Coordinate and direct home nursing care according to physician orders
• Supervise and evaluate Home Health Aides and LPNs/LVNs
• Conduct home visits for patient assessments and care
• Document patient care activities and maintain accurate records
• Travel to patients' homes within a 75-mile radius

What we need to see

• Active RN license in California
• CPR and BLS certifications
• Minimum 1 year of Hospice or Adult Home Health experience
• Proficiency with HomeCare HomeBase (HCHB) within the last 6 months
• OASIS proficiency
• Willingness to travel up to 75 miles from home clinic
