Join our team as a Physical Therapist in Home Health, where you'll provide essential care to patients in their homes.

What you'll be doing

• Conduct patient assessments and develop individualized care plans
• Perform skilled home visits, managing a caseload of 6-8 patients per day
• Educate patients and families on home exercise programs and safety measures
• Utilize various medical equipment and devices for patient care
• Document patient progress and maintain accurate records
• Collaborate with healthcare teams to ensure comprehensive patient care

What we need to see

• State license as a Physical Therapist
• BLS certification
• Minimum of 1 year of home health experience
• Strong assessment skills
• Knowledge of cardiopulmonary and circulatory systems
• Understanding of safety and injury prevention
