Join our team as a Physical Therapist in Home Health, where you'll make a meaningful impact on patients' lives in their own homes.

What you'll be doing

• Conduct home health visits for patients with various conditions
• Perform assessments and develop home exercise programs
• Educate patients and families on care and safety
• Document patient progress and maintain accurate records
• Utilize medical equipment and devices during therapy sessions
• Self-schedule visits based on office needs

What we need to see

• State licensed Physical Therapist
• Current BLS certification
• At least 2 years of home health experience
• Valid driver's license and car for home visits
• COVID vaccinated or exemption form on file
• Flu shot or declination with mandatory mask wearing

Ways to stand out from the crowd

• OASIS training
• Experience with cardiopulmonary, geriatrics, neuro, ortho, and oncology patients
