Social Worker - DH

Join our team as a Social Worker Care Manager, where you'll play a crucial role in patient assessment and care coordination.

What you'll be doing

• Perform comprehensive assessments on patient admission
• Screen patients for high-risk indicators to ensure appropriate services
• Coordinate multidisciplinary care at intra- and inter-hospital levels
• Conduct transitional planning activities including patient education and aftercare arrangements
• Communicate relevant patient information to health team members
• Document patient interactions per department requirements
• Participate in performance improvement activities

What we need to see

• Master's degree in Social Work
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in New York State
• At least 1 year of experience in an acute care hospital setting
• Patient Review Instrument Certification (PRI) within 90 days of hire
