What you'll be doing

• Provide therapy services to patients in their homes
• Conduct assessments and develop individualized treatment plans
• Document patient progress and adjust care plans as needed
• Collaborate with healthcare professionals to ensure comprehensive care
• Manage a caseload of patients while meeting visit requirements

What we need to see

• Oasis experience is required
• Active CA license preferred
• At least 1 year of Home Health experience preferred, but candidates without experience will be considered

Ways to stand out from the crowd

• Possession of a CA license in hand
• Previous Home Health experience
