Join our team as a Home Health Physical Therapist, providing essential rehabilitation services in Medford, OR.

What you'll be doing

• Deliver one-on-one therapy to home health patients
• Manage a caseload of up to 6 patients per day
• Utilize EPIC/Oasis for patient charting
• Ensure compliance with COVID vaccination requirements
• Coordinate with clients to minimize travel time

What we need to see

• Licensed Physical Therapist
• BCLS certification
• At least 1 year of skilled home health experience
• Oasis experience
• Primary series COVID vaccination and one booster

Ways to stand out from the crowd

• Experience with EPIC charting
