Therapy/Rehabilitation-OT Home Health

Join our team as an Occupational Therapist in Home Health, where you'll make a meaningful impact on patients' lives in their own homes.

What you'll be doing

• Conduct skilled home care visits for a variety of patients
• Manage a caseload of 6-8 patients per day
• Document patient care using BayCare Forms and Ace Timesheet system
• Self-schedule visits based on office needs
• Collaborate with healthcare teams to optimize patient outcomes

What we need to see

• Home health experience
• BLS certification
• At least 1 year of experience as an Occupational Therapist
• Valid driver's license and reliable transportation for home visits
• COVID vaccination or approved exemption
• Flu shot or declination with mandatory mask wearing
