Registered Nurse-Case Manager

Join our team as a Registered Nurse - Case Manager, where you'll play a crucial role in patient care coordination and transitions.

What you'll be doing

• Coordinate care and transitions for acute care patients
• Collaborate with interdisciplinary teams to ensure timely patient progression
• Conduct initial and continued assessments to determine patient needs
• Lead the development of comprehensive care coordination plans
• Monitor length of stay and manage transitions to appropriate care levels
• Document patient interactions and care plans in electronic health records
• Identify and address barriers to care progression

What we need to see

• Current CA Registered Nurse license
• BLS certification
• At least 5 years of RN Case Management experience
• Inpatient Acute Case Management experience
