Registered Nurse-Case Manager

Join our team as a Registered Nurse - Case Manager, where you'll play a crucial role in patient care coordination and transitions.

What you'll be doing

• Coordinate care and transitions for patients throughout the acute care experience
• Collaborate with physicians, social workers, and nursing staff to ensure timely patient progression
• Conduct initial and ongoing assessments to determine patient risk levels and care needs
• Lead the healthcare team in developing effective care coordination plans
• Monitor patient length of stay and collaborate on discharge planning
• Document patient interactions and care plans in the electronic health record
• Identify and address barriers to care progression and transitions

What we need to see

• Active CA RN license
• Current BLS certification
• At least 5 years of Registered Nurse Case Management experience
• Inpatient Acute Case Management experience
