Registered Nurse-Case Manager

Join our team as a Registered Nurse - Case Manager, where you'll play a crucial role in patient care coordination and transitions.

What you'll be doing

• Coordinate care and transitions for patients throughout the acute care experience
• Collaborate with physicians, social workers, and nursing staff to ensure timely patient progression
• Conduct initial and ongoing assessments to determine patient risk levels for readmission
• Lead the healthcare team in developing effective care coordination plans
• Monitor length of stay and collaborate with financial counselors on inpatient stay denials
• Formulate transition plans in collaboration with patients, families, and healthcare teams
• Identify and address barriers to care progression and transitions

What we need to see

• Active California Registered Nurse license
• Current BLS certification
• Minimum of 5 years of RN Case Management experience
• Experience in Inpatient Acute Case Management
