Registered Nurse-Case Manager

Join our team as a Registered Nurse - Case Manager, where you'll play a crucial role in patient care coordination and transitions.

What you'll be doing

• Coordinate care and transitions for patients throughout the acute care experience
• Collaborate with physicians, social workers, and nursing staff to ensure timely patient progression
• Conduct initial and ongoing assessments to determine patient needs and risk levels
• Lead the healthcare team in developing effective care coordination plans
• Monitor patient length of stay and collaborate on transition planning
• Document patient interactions and care plans in the electronic health record

What we need to see

• Active California RN license
• Current BLS certification
• Minimum of 5 years of RN Case Management experience
• Experience in inpatient acute case management
• Familiarity with Epic electronic health record system
